Add <limits.h> to the include files handled by "cache.h", and remove extraneous #include directives from various .c files. The rule is that "cache.h" gets all the basic stuff, so that we'll have as few system dependencies as possible.

This moves the private "say()" function to pull.c, renames it to "pull_say()", and introduces a global variable "get_verbosely" that makes the pull backends report what they fetch. Currently pull() calls fetch() without checking whether we have the wanted object but all of the existing fetch() implementations perform this check and return success themselves. This patch moves the check to the caller. I will be sending a trivial git-local-pull which depends on this in the next message. This patch adds three similar and related programs. http-pull downloads objects from an HTTP server; rpull downloads objects by using ssh and rpush on the other side; and rpush uploads objects by using ssh and rpull on the other side. The algorithm should be sufficient to make the network throughput required depend only on how much content is new, not at all on how much content the repository contains. The combination should enable people to have remote repositories by way of ssh login for authenticated users and HTTP for anonymous access.
