refspec: rename struct refspec to struct refspec_item
In preparation for introducing an abstraction around a collection of
refspecs (much like how a 'struct pathspec' is a collection of 'struct
pathspec_item's) rename the existing 'struct refspec' to 'struct
refspec_item'.
